Surfaced 2026-06-16 from Dad's CVS prescription history — these look like his pre-existing home providers, so continuity relationships may already exist. ⚡ Active outreach — two calls to make

(a) Tiffany Joseph (psych) — get the actual prescribing history (doses, quantities, refill cadence — to resolve the conflicting accounts of how he was using it), ask whether she can resume his care via telehealth or refer to a kindling-literate psychiatrist, and confirm the alprazolam is discontinued at the prescriber level (it's been removed at CVS, but the underlying Rx may still exist).

(b) Cardiology — re-authorize the lapsing cardiac Rx's: clopidogrel (half his post-Watchman DAPT — stroke prevention) and metoprolol both show "needs new Rx." These cannot lapse. Route through his cardiology practice (Markovic / Bay Area Heart Center) or whoever will own cardiac refills.

TRT has no in-house neurology, so this is an outside referral — ideally a movement-disorder specialist who takes his insurance. Tics have escalated to a raw tongue, and the tic regimen was never built intentionally (the benzo was quietly doing that job). A full visit-prep doc is ready to walk in with.

Three highest-priority providers to identify first. PCP coordinates everyone. Cardiologist owns the DAPT step-down. Outpatient psychiatrist owns med continuity and must be fluent in benzodiazepine dependence, kindling, and addiction medicine (ABPM board-certification is a strong filter — covers both the dependence and the possible-misuse picture). Several of these may already exist — see Dad's existing home doctors above; Tiffany Joseph could fill the psych role if she's aligned with the off-benzo, kindling-aware plan.
